Job summary

Spectrum Community Health CIC is seeking a Registered Nurse (Band 5) to join our Primary Care and clinical DART team. You will support physical health assessments of patients, plan care and deliver a range of treatments/interventions.

You will work within a multidisciplinary team, coordinating with Mental Health and Substance Misuse services to optimise on-site care and meet the needs of vulnerable patients with complex health requirements.

Qualifications

• Registered Nurse (Band 5) with current practice registration.
• Experience in primary care and health assessments preferred.
• Ability to work in a multidisciplinary team and handle flexible hours.

Responsibilities

• Undertake health assessments and diagnostic tests as needed.
• Plan care and provide a range of treatments/interventions.
• Support patients to address health, behavioural and social welfare issues.
• Manage priorities and respond to emergencies.

• Working closely with our management team and key stakeholders, you will contribute to the delivery of a comprehensive primary care health service.
• You will be part of a multi-disciplinary team, undertaking physical health assessments of patients, planning care and providing a range of health treatments/interventions.
• You will support patients to address issues that affect their own health, behaviour and social welfare
• Manage competing priorities and react to unforeseen circumstances, incidents and emergency situations.

As a Registered Nurse (Band 5), you'll be an integral part of our Primary Care and clinical DART team, supporting some of our most vulnerable patients and planning care for people with complex and challenging health needs. You'll be confident in all aspects of primary care delivery - including undertaking health assessments and performing diagnostic tests - and able to work flexibly to meet the needs of our service. We're looking for a candidate with lots of energy and an enthusiastic, "can do" attitude. As part of a multidisciplinary team, you'll be working closely with Mental Health and Substance Misuse services to optimise on-site care and you'll need to form working relationships with a wide range of partners.
