Position Summary

Baptist Health Brookwood Hospital

Position Summary: Administers patient care in a hospital or inpatient/observation setting where an acute care patient receives active treatment for an injury, illness, medical condition, or post-intervention assessment.

Responsibilities

Essential Functions:

  1. Assesses patient needs and develops/revises an individualized care plan based on patient responses, evaluating progress toward expected outcomes.
  2. Respects diversity by building respectful relationships with all team members and patients.
  3. Acts as a patient and family advocate.
  4. Provides age-appropriate care, demonstrating necessary knowledge and skills.
  5. Serves as a preceptor, charge nurse, educator, and/or nurse clinician.
  6. Collaborates with medical staff and interdisciplinary team for effective planning and management.
  7. Supports hospital and nursing department goals and strategies as a role model.
  8. Demonstrates understanding of growth and development principles, assesses patient status, and interprets data to meet age-specific needs.
  9. Coordinates care and delegates tasks appropriately.
  10. Documents patient care accurately and meeting regulatory standards, including assessments, education, medication administration, treatments, and safety.
  11. Demonstrates competency in nursing skills and equipment use.
  12. Prioritizes patient care based on Evidence-Based Practice Standards.
  13. Identifies and resolves problems effectively.
  14. Delegates tasks considering patient needs and team member capabilities.
  15. Communicates patient condition and unit concerns effectively to team members.
  16. Provides compassionate, therapeutic care environment for patients and families.
  17. Maintains awareness of legal issues and patient rights.
  18. Supports new team members and students through collaboration with education and leadership teams.
  19. Maintains attendance and complies with hospital policies, including ADA, FMLA, and other standards.

Hospital Care at Home (Program Functions – Patient Care Hub)

Virtual Hospital Care: Administers patient care via TeleHealth technology, assessing needs, developing care plans, and evaluating progress. Capable of conducting virtual visits, assessing symptoms, and escalating issues as needed. Collaborates with in-home staff and demonstrates age-appropriate care skills.

In-home Nursing:

Provides direct care in patients' homes, assessing needs, contributing to care plans, and traveling within designated regions. Works autonomously, using virtual technology for communication, and transports items and medications as required.

Qualifications

Education/Training: Graduate of an approved nursing school. Meets all competency requirements for Orlando Health and the specific department.

Licensure/Certification: Current Alabama RN license or valid eNLC multistate license, BLS/Healthcare Provider certification. Additional certifications like ACLS, NRP, PALS, TNCC may be required for specific units. Valid driver’s license and reliable transportation required for in-home nursing roles.

Experience: Minimum 1-year nursing experience required, or as approved by leadership. For Hospital Care at Home, at least 2 years of acute care experience is required.
