Job description

As a Registered Nurse you will play a crucialrole in providing comfort and support to patients and their families. You willwork closely with our multidisciplinary team to develop and implement careplans that prioritise symptom management, pain control, and emotional support.

Main duties of the job

Assessing and monitoring patients physical emotional and spiritual cultural needs

Administering medications and treatments asprescribed

Ensure a high standard of writtendocumentation is maintained.

Providingskilled nursing care, including wound care and catheter management

Collaboratingwith physicians, social workers, chaplains, and other members of themultidisciplinary team to ensure holistic care

Advocating for patients preferences andgoals of care

Liaise withvoluntary and statutory agencies to ensure continuity of care from the hospitalto the community, or continuing care setting.

Dutiesand Responsibilities

Clinical

1 Assess, plan, implement and evaluatecare for patients in a way that reflects the Hospice philosophy.

2Ensure a high standard of writtendocumentation is maintained.

3Promote a multi-disciplinary approach tocare by ensuring effective communication is maintained between all staff,patients and their families.

4Liaise with voluntary and statutoryagencies to ensure continuity of care from the hospital to the community, orcontinuing care setting.

5Ensures the cultural and spiritual needsof patients are recognised and accommodated within resources available.

6Develop knowledge in pain and symptomcontrol.

7Safely set up and maintain S.C and I.Vpumps and administer all medications in accordance with Hospice and NMCguidelines.

8To be responsible, in conjunction withthe Team Leader and Ward Manager, for maintaining a safe environment forpatients, visitors and other staff on the ward.

9Works as part of the multidisciplinaryteam in supporting patients and their carers during the delivery of bad news,caring for the terminally ill patients and the event of death.

10Manage calls to the Hospice referringonto other professionals as appropriate.

11Assess the needs of families and carerswho may require bereavement support services.

12To work within Harlington Hospicepolicies (local) and guidelines.

13Work to meet the objectives of theHospices Nursing and Midwifery strategy.

Managerial

14Co-ordinate the care on the ward for agiven shift in the absence of the Team Leader or Ward Manager.

15Ensure, by close liaison with TeamLeader or Ward Manager, adequate staff of appropriate skill level to meet theneeds of the Hospice within the constraints of the budget.

16Ensure, by close liaison with the TeamLeader or Ward Manager, adequate provision and economical use of Hospice provisionsand stores.

Educational

17Act as a mentor for learner nurses/NVQtrainees and a preceptor for junior staff nurses.

18Contribute to the education programmewithin the ward.

19Ensure patients and familieseducational and information needs are met.

20Ensure one is educationally andprofessionally updated and meet the criteria of the NMC PREP document.

21Participate in clinical audit programmesor research projects as directed by the Ward Manager.

Professional

22Ensure continued, effective registrationwith NMC.

23Be aware of the NMC Code of ProfessionalConduct and be accountable for own practice.

24Be aware of the NMC Scope ofProfessional Practice and ensure competency to undertake duties allocated.

25Ensure professional portfolio is keptand updated in accordance with NMC recommendations.

26To assist trained staff in dealing withpatients/relatives/visitors who are distressed or anxious, and report to nursein charge.

27To deal with initial complaints andconflict (if approached directly by a patient/relative/visitor) as required, ina polite, calm manner and report to the nurse in charge.

28Any other duties as may be required.

The post holder mustmaintain the confidentiality of information about patients, staff and otherhealth service business in accordance with the Data Protection Act of 1984.
