North Cheshire and Mersey NHS Foundation Trust in Warrington is recruiting a Band 4 Nursing Associate to join our Community Nursing Team, delivering patient-centred care in people’s homes alongside Registered Nurses and the wider MDT.

You will perform a wide range of delegated clinical tasks within your scope of practice, monitor patients’ health, recognise changes, and escalate concerns to the Registered Nurse. Flexible working options will be considered to meet service needs.

Main duties of the job

As a Nursing Associate, you will work alongside Registered Nurses and the wider multi-professional team to provide safe, compassionate and person-centred care. You will deliver a wide range of delegated clinical care within your scope of practice, promoting health, independence, dignity and wellbeing. You will work independently where appropriate, while recognising your professional boundaries and escalating concerns or changes in a persons condition to the Registered Nurse or other appropriate professional. Your role will include undertaking clinical observations, monitoring blood glucose, nutrition and hydration, supporting medication administration, wound care, catheter care, venepuncture and cannulation where trained and competent, and contributing to assessment, care planning and evaluation. You will support individuals with complex and long-term conditions, end-of-life care, dementia and other health needs, ensuring their physical, emotional, psychological, social, cultural and spiritual needs are considered. You will maintain accurate electronic records, follow safeguarding, consent, Mental Capacity Act, infection prevention and information governance requirements, and report incidents or concerns appropriately. You will communicate effectively with patients, families, carers and colleagues, demonstrating compassion, empathy and respect at all times.

Clinical Responsibilities

• Deliver high quality, compassionate care under the direction of a registered Nurse (or other registered professional dependent on the setting) with a focus on promoting health and independence.
• Be responsible and accountable for own practice, working within limits of competence and within professional boundaries while raising concerns to a registered care professional or appropriate person as and when required.
• Carry out specific delegated clinical tasks and responsibilities to a high standard and competency which promotes excellent patient care and continued partnership working.
• Where appropriate work without direct supervision, at times delivering care independently in line with the individuals plan of care, within designated parameters of practice of the Nursing associate role, accessing clinical and care advice when needed while providing feedback on progress against the care plan.
• Maintain understanding of all elements of the nursing process and be able to assist the registered nurse in on‑going assessment, planning, management, and evaluation of care.
• Support individuals with all aspects of care including daily living, providing person‑centred care and promoting health and independence i.e. Making Every Contact Count.
• Manage and monitor the condition and health needs of individuals within their care following administration of medication, and to escalation as necessary.
• Undertake tasks that are relevant to the speciality including cannulation, venepuncture and cannulation, including administration of medication as per trust policy.
• Perform and record clinical observation including blood pressures, temperature, respirations, pulse, peak and oximetry measurements and any additional requirements within speciality and competence.
• Accurately record nutritional and fluid intake to identify signs of dehydration or fluid retention and escalation as necessary.
• Use contemporary assessment tools e.g. Waterlow and nutritional assessments.
• Assist with feeding and drinking and support the delivery of artificial nutrition and hydration using oral and enteral routes as set out by Dietician and/or SLT Team.
• Measure and interpret blood glucose levels, recognise emergency situations, administer basic physical first aid, including BLS and observe and monitor comfort, pain levels, rest and sleep patterns.
• Use appropriate positioning and pressure relieving techniques, and pressure relieving / manual handling equipment.
• Monitor wounds and undertake wound care using appropriate evidence‑based techniques, use aseptic non‑touch techniques, care for, manage and insert catheters for all genders including all associated documentation.
• Review preferences and care priorities of the dying person and their family and carers, and ensure changes are communicated as appropriate, providing care for the deceased person and the bereaved respecting cultural requirements and protocols.
