Key Responsibilities:

  • Medications management: Ensure safe and timely medication handling, including ordering, stock control, dispensing, administering, controlled drugs management, and reporting.
  • Create and update Care Plans and risk assessments: Develop personalized care plans in collaboration with residents, families, and healthcare teams.
  • Monitor resident health: Conduct regular assessments, monitoring, and reporting, adopting a holistic approach.
  • Assist with activities of daily living: Support residents with mobility, bathing, dressing, and eating, following care plans and risk assessments.
  • Demonstrate a person-centred approach: Provide compassionate care to residents and their families.
  • Collaborate and communicate with teams: Work effectively with healthcare professionals.
  • Lead and manage teams: Act as an autonomous practitioner when needed, leading shifts effectively.
  • Communicate effectively at all levels: Ensure clear and transparent communication and reporting.
  • Ensure compliance and safety: Maintain high standards of care, safety, and infection control, with proper documentation.
