Job Description
As a Mental Health Social Worker, you will maintain appropriate collaboration with a multidisciplinary core team of professionals working directly with one or more individual young people with complex needs. You will work alongside the Tier 3.5 Clinical and Social teams to implement bespoke Mental Health Therapeutic Interventions to ensure the best outcomes for our young people.
- Work with a team to support young people in the Tier 3.5 Service.
- Provide direct therapeutic support to young people and their families.
- Understand their needs and promote self-esteem, independence, resilience, and positive mental health.
- Keep intervention and care plans up to date and share them with the team.
- Collaborate with Clinical Leads to ensure the effectiveness of Clinical and Social Teams.
- Deliver intervention work, document and record necessary information, and provide feedback.
- Ensure the Great Minds Together model is implemented, and progress is recorded accurately.
- Attend meetings with the Psychology team to share information and data.
- Help develop and implement Support Plans, Behaviour Analysis reports, Risk Assessments, and Care Plans.
- Ensure all staff and volunteers follow organizational policies and processes.
- Support the rota management system to meet the needs of young people.
- Coordinate tasks, events, and workload using an internal management system.
Qualifications & Experience…
Qualifications (Essential):
- To be educated to a good standard
- Social Work Qualification with valid registration to Social Work England
Experience (Essential):
- Experience working with young people (particularly those that may present with distressed behaviour) in family situations, school, children’s residential establishments or in voluntary activities with young groups.
- Experience in working with young people that have a learning disability, Autism and/or have experienced Trauma (including Trauma from unmet needs).

- Excellent communication skills - both written and spoken
- Good people skills and an ability to form and maintain positive relationships.
- Be approachable and reflective with a positive solution focused approach.
- Have a passion for working with children and young people and supporting the learning of others. This will be evident in your approach and enthusiasm for your work.
- Able to balance perspectives and politely challenge professionals in a Multidisciplinary Teams arena whilst maintaining positive relationships with them. Always ensuring the young person’s and their family’s best interests are at the centre of everything we do.
