The Forward Trust is recruiting a RECONNECT Support Worker to assist individuals leaving prison in Bedfordshire and Milton Keynes. This permanent role focuses on health, wellbeing and community reintegration within NHS:commissioned services.
You will assess needs, develop personalised care plans, and support clients through referral access to GPs and community services with ongoing post-release contact for up to six months.

Salary: A GBP 26,000 per annum

This is an exciting opportunity to use your skills, experience and understanding to help people navigate the challenges of returning to the community and connect with the support they need.
RECONNECT is designed to help people leaving prison access and maintain the health, wellbeing and community support they need.
You will work with people across HMP Bedford, HMP Woodhill, Yarls Wood IRC and the wider Bedfordshire and Buckinghamshire community.
