Job summary

Change Grow Live is seeking a Through the Gate Worker to support individuals preparing for release from HMP Channings Wood. You’ll help access treatment, healthcare and community services, and assist in building plans for a successful reintegration into the community.

The role emphasises resilience, relationship-building and a commitment to developing skills through training and support. Applicants should be ready to work 37.5 hours per week in a permanent position on-site.

Qualifications

• Strong people skills and ability to build positive relationships.
• Ability to communicate effectively with colleagues and service users.
• Organised and able to manage competing priorities while working in a secure setting.

Responsibilities

• Support individuals during the 12 weeks leading up to their release from prison.
• Develop personalised resettlement and recovery plans.
• Coordinate referrals to healthcare, treatment, and community services.
• Work closely with prison staff, healthcare teams, resettlement teams and external partner agencies.
• Advocate for individuals to access the support they need.
• Promote recovery, resilience, social reintegration and independence.
• Contribute to risk management and safeguarding processes where required.
