Support Worker

Location: Fareham, Hampshire

Hours: 37.5 hours per week

Salary: £12.71 per hour

We have an exciting opportunity for someone dedicated, passionate, and driven to join our caregiver team at Uplands House Care Home in Fareham. Uplands is a Nurse-led service for people with mental and physical health needs, located on the outskirts of Fareham.

Ideally, you will have experience in the care sector, specifically working with individuals with mental and physical health needs, be familiar with safeguarding, and have a clear understanding of personal development plans.

Responsibilities:
  • Provide care and support to residents, following their support plans to enhance their quality of life and wellbeing.
  • Support residents with their physical and mental health needs, including personal care.
  • Promote physical wellbeing through health checks and healthy lifestyle choices.
  • Assist in developing person-centered and inclusive support plans with the nursing team.
  • Encourage residents' independence and involvement in daily activities.
  • Support residents in community integration and social activities.
  • Build relationships with residents to support their recovery and create a therapeutic environment.
  • Accompany residents on outings and shopping trips.
  • Participate in meal preparation and serving, respecting personal preferences and dietary needs.
  • Engage residents in scheduled activities aligned with their recovery plans.
  • Act as a key worker for residents, supporting their recovery pathways.
  • Maintain respect, acceptance, and tolerance, ensuring dignity and needs are met.

This list is not exhaustive; additional duties may be assigned following discussions with your line manager.
