Job summary

Responsibilities

• Provide support and guidance to all participants registered onto the project.
• Develop ongoing action plans with participants and review regularly.
• Provide one-to-one support and group activities to aid recovery.
• Work alongside the team structure to deliver services and outcomes for the scheme.
• Achieve project targets and outcomes set by Welsh Government.
• Support the team leader and referral agents in raising awareness of the service.
• Work in partnership with other practitioners to deliver interventions and support for mental health and/or substance misuse.
• Provide support and supervision to volunteers.
