Job summary

Creative Support is recruiting for its Floating Support Service in Bishop Auckland and East Durham. The role provides practical and emotional support to individuals with mental health needs in their homes or in the community.

You'll identify social inclusion opportunities and work recovery-focused with clients, families, and professionals, ensuring a coordinated service. Training is provided; drivers with access to a car are essential. This is a full-time/part-time role located in the UK.

Qualifications

• Experience is not mandatory for contracted roles; comprehensive ongoing training is provided.

Responsibilities

• Identify and access opportunities for social inclusion with clients.
• Work with clients in a recovery-focused manner according to individual support plans.
• Collaborate with clients, families, and professionals to deliver coordinated support.
• Ensure a consistent and coordinated service across the team.
• Assist the Project Manager with day-to-day operations of the service.
