Job summary

Leeds Community Healthcare NHS Trust in Leeds is recruiting a Band 5 Staff Nurse to work in the Short Term Care Beds service. You will provide person-centred care and rehabilitation, supporting patients on their recovery journey across a 24/7 operation.

Join a multidisciplinary team including RNs, HCWs, occupational therapists, physiotherapists, dietitians and GP practice. You’ll assess care needs, deliver care plans and drive improvements while maintaining high governance standards.

Qualifications

• Qualified nurse with current NMC registration.
• Experience in community nursing and rehabilitation.
• Ability to work within a multidisciplinary team.
• Excellent communication and safeguarding skills.

Responsibilities

• Assess patients' care needs and develop care plans.
• Implement, monitor and evaluate programmes of care.
• Work across varied shift patterns to ensure 24/7 care.
