This role is to work in the Diagnostics Service within the Older People’s Care Unit providing triage and pre-assessment of all the new referrals that are received by the team.

This is a vacancy for an existing Band 6 Nurse

The role will take responsibility for the receipt of referrals made either internally or by external organisations, ensuring all patients referred for an assessment are contacted and triaged and referred for a CT/MRI scan where required, before the initial assessment takes place.

The postholder will serve as the primary contact for the service, handling calls from referrers, patients, and carers who may have urgent queries that cannot be delayed until their care coordinator becomes available.

This role is working within Nottinghamshire Healthcare NHS Foundation Trust within the Mental Health Services for Older People directorate.

The successful candidate will work within the Diagnostic service and will review/accept referrals for individuals over the age of 65 with cognitive impairment for the MAS Service and under 65 years for the Young Onset Dementia Service.

To screen referrals for appropriateness and complete pre-assessment for all patients.

Identifying reversible causes of memory problems, such as Vitamin deficiencies or medication side effects, ensuring these are addressed before or alongside dementia diagnosis.

Responsible for ongoing robust clinical risk assessment of individuals.

Pont of contact for patients/carers/referrers to absence of their care co-ordinator

To hold adhoc outpatient clinics and cover short term sickness absence at short notice.

To liaise with the MDT to explore symptoms and causes

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

Please note that this post does not meet the pay level required for a Skilled worker visa.

Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.
