Keynsham Health Centre in BS31 1AF is seeking a Community Nurse Band 5 to deliver high-quality, person-centred care in patients' homes. You will work with GPs and the multidisciplinary team, assess needs, plan and deliver care, promote independence, and contribute to service development.

As a Community Nurse Band 5, you will deliver high-quality, person centred nursing care to patients in their own homes, working in line with national and local policies, professional standards, and the NMC Code. You will work closely with patients, families, GPs, and multidisciplinary colleagues to assess needs, plan and deliver care, promote independence, and support positive health outcomes. You will be a NMC/1stlevel registered Nurse, you will have the ability to work both independently and also as part of a wider multidisciplinary team. It is expected that you should have strong communication and organisation skills and a flexible approach which will ensure you deliver positive experiences to patients at every stage of their healthcare journey. You will have a full UK driving license with access to a car for work purposes with Business insurance for the vehicle used. You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
