Job summary

Cheshire and Wirral Partnership NHS Foundation Trust is recruiting a Social Worker within Integrated Services for adults with disabilities at the Millennium Centre, Leasowe. You will support people with complex needs and their carers to achieve meaningful outcomes in an integrated, innovative service.

You will carry out Care Act assessments, develop personalised support plans, complete mental capacity assessments and lead safeguarding enquiries. SWE registration is required.

Our team works with people with complex needs to support them, and their carers, to get the outcomes they need. CWP is at the forefront of health and social care integration and this service lends itself to developing new, innovative ways of working together.

You will be a person-centered practitioner and will have experience of carrying out Care Act assessments, developing personalized support plans, completing mental capacity assessments, and taking a lead on safeguarding enquiries. A good understanding of asset and strength-based approaches to assessment and support planning is vital.
