Job summary

Leicestershire County Council is hiring a Reablement Team Leader (Charnwood locality) to support the Home Care Assessment Reablement team in the community and hospital settings. You will lead first visits, conduct risk assessments and guide a team of Reablement Support Workers to help service users regain independence.

The role requires NVQ 3 or equivalent, strong IT skills, and the ability to travel across the County.

Qualifications

• To apply for this post, you must have relevant experience working in adult social care settings and a good understanding of the Council's Adult Social Care strategies.
• An NVQ 3 or equivalent and relevant experience working with adults in a social care setting is essential; a good understanding of the relevant regulations and service frameworks.
• Ability to use Information Technology applications (e.g., Microsoft Outlook, Word, Excel and databases) and to identify and record critical information with problem solving.
• Ability to prioritise and manage own workload and meet deadlines.
• Ability to communicate clearly and accurately, both orally and in writing, with a wide range of people, using negotiation and persuasive skills.
• Willingness to travel and work from various locations across the County; access to a vehicle is required.

Responsibilities

• Undertake first visits and carry out risk assessments.
• Provide day-to-day line management support to Reablement Support Workers.
• Undertake Care Act Assessments where eligible need has been identified.
• Deliver the Reablement plan in collaboration with Occupational Therapists and Physiotherapists.
• Support service users to regain independence in daily living activities.
