Job Description:

You will be working as a vital Reablement Support Worker within the Intermediate Care Service within Bury Council. This position is 100% community focussed within Bury and surrounding areas. These positions support adults in medical and/or mental health reablement within their own homes, especially those recently discharged from hospital or reablement units without a strong support network, aiming to restore independence.

Essential requirements include being a driver with access to a car and comfort working in clients' homes.

These are full-time positions with a split shift pattern to meet individual needs: Week 1: Monday, Tuesday, Saturday, Sunday; Week 2: Wednesday, Thursday, Friday. Hours are 8.30am-1pm and 4.30-9.30pm.

· Provide person-centred, therapeutic, and creative solutions to support recovery and independence.

· Deliver, evaluate, and adjust reablement programmes to achieve goals within recovery timelines.

· Collaborate with a multi-disciplinary team across health, social care, and community services.

· Maintain a safe, stimulating environment for clients in reablement.

Requirements

Experience in home care is preferred, with qualification to at least Level 2 or willingness to develop skills. Experience with moving and positioning adults and medication distribution is essential. You should be self-motivated, energetic, enthusiastic, and able to encourage positive behaviour changes. Good communication skills and proficiency in technology for care planning and reporting are necessary.

This role reports to a Community Reablement Manager/Team Lead, employed by Bury Council at Band 7, with benefits including generous holiday entitlement and allowances.
