You will be working as a vital Reablement Support Worker within the Intermediate Care Service within Bury Council. This position is 100% community-focused within Bury and surrounding areas. The role involves supporting adults in medical and/or mental health reablement at their own homes, particularly those recently discharged from hospital or reablement units who may lack family or social support in the short term to regain independence.
It is essential to be a driver with access to a car and be comfortable working in clients' homes.
This is a full-time position with a split shift pattern aligning with client needs: Week 1: Monday, Tuesday, Saturday, Sunday; Week 2: Wednesday, Thursday, Friday. Hours are 8:30 am - 1:00 pm and 4:30 pm - 9:30 pm.
We seek a support worker experienced in home care, ideally qualified to Level 2 or willing to develop skills. Experience with moving and positioning adults and medication distribution is essential. You should be self-motivated, hardworking, and capable of managing your own caseload within a small team. Physical and mental well-being are crucial due to the busy nature of the service. An energetic, enthusiastic attitude and the ability to encourage positive behavioral changes are important.
Good communication skills and proficiency in technology for care planning and reporting are required.
This role reports to a Community Reablement Manager/Team Lead and is employed by Bury Council on a Band 7 salary of £25,584 per annum (£13.26 per hour). Benefits include generous holiday entitlement and a benefits allowance.
