Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting a Healthcare Support Worker to provide rehabilitation support to adults in their homes and community settings. You will work with therapists and a multidisciplinary team to promote independence and wellbeing.

The role requires compassionate communication, flexibility and a commitment to person-centred care, with full training provided and options to develop within the integrated community services.

Qualifications

• NVQ in Health Care or equivalent, or willingness to commit to training.
• Current driving licence and access to a vehicle for work purposes.

• Demonstrates effective written and verbal communication skills.

Join our Integrated Community Rehabilitation Team as a Healthcare Support Worker and make a real difference to people's lives. Working alongside therapists, and Portsmouth City Council colleagues, you will provide rehabilitation support to adults within their own homes and community settings, helping them regain independence, improve their wellbeing, and achieve their personal goals. You will assist with rehabilitation programmes, support patients with daily activities, monitor progress, and work closely with the multidisciplinary team to deliver high-quality person-centred care. This is a rewarding role requiring excellent communication skills, compassion, flexibility, and a commitment to supporting people to live independently. The service hours are 7am to 10pm and shift patten is 7am-3pm or 3pm-10pm with alternate weekend working. At present, the Trust is unable to offer Skilled Worker visa sponsorship for this post. Applicants must have the right to work in the UK at the time of application.Main duties, tasks & skills required

• Undertake evidence-based interventions with patients, as agreed with a therapist/Community Nurse to ensure good clinical outcomes. The post holder will be required to work with the patient to gather information, highlighting the need for further assessment, and completing a clinical record of the intervention including the outcome.
• Actively promote recovery-focussed care with patients, in conjunction with other health professionals.
• Support patients with their rehabilitation and recovery, respecting them as individuals, supporting them and understanding their right to express their needs.
• Facilitate and lead patient-focus groups and activities.
• Use appropriate approved conflict resolution and management of aggression techniques (e.g., PMVA), to de-escalate challenging situations as required for the role.
• Undertake clinical and environmental risk assessments and document key risk areas, seeking guidance from more senior staff when dealing with cases indicating a high level of risk.
• Communicate effectively with patients, carers, colleagues, in order to motivate patients to engage in the intervention.
• Ability to communicate with patients' carers and clients where motivation maybe low and engagement is difficult, necessitating the use of motivational and persuasive skills where there maybe barriers to understanding.
• Participate in the administrative and professional responsibilities team., The post holder will have responsibility for participating in the delivery of patient care under the supervision of the Community Team.
• Work under supervision following written care plans and care pathways devised by the Registered Nurse.
• Follow prescribed care plans and treatment protocols.
• Follow HPCT Policies and Procedures.
• Has knowledge and skills to carry out prescribed tasks and will report to Registered Nurse any variances.
• Delegated responsibility for Patient Support Teams administrative tasks.
• Provide general administration support to the team.

This role will demonstrate excellent communication skills, using them to engage, persuade or negotiate with clients with complex, at times challenging needs to ensure they are provided with the most appropriate care and support.

The Team consists of Occupational Therapists, Physiotherapists, Nurses, Doctors, Consultants, Social Workers, and Healthcare Assistants who work together in an integrated way to provide co-ordinated, clinical support to promote independence and to improve quality of life.
