Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting a Healthcare Support Worker to provide rehabilitation support to adults in their homes and community settings. You will work with therapists and a multidisciplinary team to promote independence and wellbeing.

The role requires compassionate communication, flexibility and a commitment to person-centred care, with full training provided and options to develop within the integrated community services.

Qualifications

• NVQ in Health Care or equivalent, or willingness to commit to training.
• Current driving licence and access to a vehicle for work purposes.
• Experience in an administrative or healthcare support role preferred.
• Computer literacy and ability to use basic software systems.
• Demonstrates effective written and verbal communication skills.

Responsibilities

• Undertake evidence-based interventions with patients under therapist guidance.
• Promote recovery-focused care with patients and other health professionals.
• Support rehabilitation, encourage independence, and respect patient preferences.
• Lead patient-focus groups and activities as required.
• Record clinical information and monitor progress with supervision.
