Croydon Health Services NHS Trust is seeking a Rapid Response clinician to assess and treat patients at their own homes during crises, identifying complex needs and developing care plans with the multidisciplinary team. The role demands a high degree of autonomy and advanced clinical skills to address undifferentiated cases in the community.

Working as part of the Rapid Response team, the post-holder will provide assessment and treatment of patients, within their own home, during a crisis. The post holder will identify complex and specialist needs and will develop, implement and evaluate programmes of care. Working as a practitioner with a high degree of autonomy, the post-holder will manage undifferentiated and undiagnosed problems using advanced clinical skills by incorporating physical assessment and diagnosis and be supported to do this by senior members of the MDT. The post holder will also identify patients suitable for virtual ward monitoring and be proficient in fitting the appropriate technology following training and support from the team and will be responsible in assessing unwell patients at home on request of the virtual ward hub.
