Qualifications

Responsibilities

• Assist and promote independent social and living skills including mobility, personal care, bathing, dressing/undressing; nutrition, hydration and prompting, monitoring and administration of medication where required, in line with local policies.
• Undertake clinical tasks as delegated after training, including baseline observations and investigations, simple wound care and like-for-like dressings.
• Recognise deteriorating physical health and use clinical observations to assist in assessment; escalate using early warning score and SBAR as required.
• Ensure patients, relatives, carers and professionals are kept informed of progress following intervention and communicate complex information to meet varied understanding levels.
• Travel efficiently around the South Devon area; full driving licence and vehicle expected to carry out duties.
