Pay rate c. £33.94 per hour

Vitalis is recruiting an experienced Qualified Social Worker for an Integrated Learning Disability Team in Hackney.

  • Record actions and decisions clearly on Mosaic.
  • Work as part of a multi-agency team providing seamless health and social care services.
  • Provide advice and preventative support for adults with Learning Disabilities.
  • Respond to urgent matters, promoting safety and wellbeing of service users with learning disabilities.
  • Conduct assessments, reviews, and visits as needed, especially in emergencies.
  • Be the first point of contact for safeguarding concerns, liaising with colleagues internally and externally.
  • Ensure risk management and application of MCA principles.
  • Maintain high professional standards, including quality interventions, decision making, and adherence to Social Work England standards.
