Job description

Duty Social Worker - Integrated Learning Disability Team

Our Client Hackney Council is looking for a Duty Social Worker for their Integrated Learning Disability Team.

Role specific responsibilities:

  1. To record actions and decisions in a clear and robust way on the Mosaic.
  2. To work as a part of a multi-agency team providing seamless health and social care services.
  3. To provide direct advice and assistance when required and to offer preventative support for adults with Learning Disabilities.
  4. To respond to urgent matters raised on duty by using a range of resources to promote the safety and wellbeing of our service users with learning disabilities.
  5. To carry out an assessment/review and visit when required as a response to an emergency.
  6. To be the first point of contact for safeguarding concerns and to be able to follow up on them to ensure the safety of our service users, including liaising with both internal and external colleagues as appropriate.
  7. To ensure that the risk management is adhered to in social work practice and that principles of the MCA Act are applied where appropriate.
  8. To deliver high professional standards of social care practice including: quality interventions and decision making; agreed performance management; and adherence to the Standards of Social Work England.

What you need:

  • QSW Status - including qualification, Social Work England registration and completed ASYE.
  • A minimum of 2 years' experience.
  • Right to work in the UK.
