About the Role

We are delighted to offer an exciting opportunity for a Qualified Social Worker to join the Adults Community Mental Health Team (CMHT) in Salford. This rewarding role offers the chance to make a real and lasting difference in the lives of adults with complex mental health needs, while benefiting from a supportive and collaborative working environment.

This is a full-time, ongoing position offering an hourly rate of £30.00. You will play a vital role in promoting recovery, wellbeing, and independence through person-centred and strengths-based social work practice.

Key Responsibilities

Undertake comprehensive, strengths-based assessments in line with statutory duties and local authority frameworks.

Develop and implement person-centred care and support plans that promote independence, wellbeing, and recovery.

Manage a caseload of adults with severe and enduring mental health needs, ensuring timely reviews and effective interventions.

Work collaboratively with service users, carers, healthcare professionals, and partner agencies to deliver coordinated care.

Exercise sound professional judgement to manage risk, safeguarding concerns, and complex decision-making.

Contribute to multidisciplinary team meetings, case conferences, and discharge planning processes.

Maintain accurate and timely case records in line with Salford City Council policies and procedures.

Ensure compliance with all relevant legislation and statutory requirements, including the Care Act 2014, Mental Health Act 1983, and Safeguarding Adults procedures.

Participate in supervision, reflective practice, and continuing professional development to maintain and enhance professional standards.

Support the implementation of service improvements and council-wide initiatives to enhance mental health and wellbeing outcomes across Salford.
