Qualified Social Worker - Adults

Community Team Learning

Location: West Yorkshire

Pay rate: £35 per hour

We are looking for experienced and passionate Social Workers to join a dedicated Community Team Learning Team. You will be responsible for managing cases for which you are the named worker, intervening with legal and procedural authority to protect vulnerable people at risk of abuse or neglect, and supporting individuals, families, and groups to sustain and develop their capacities for independent living. This role involves working within a Community Mental Health Team, with a focus on making a difference to vulnerable adults. It includes home working and local authority collaboration.

In accordance with social care legislation and local government requirements, you will support the delivery of person-centered, safe, and effective social care services. Using your professional knowledge and experience, you’ll make assessments and decisions about appropriate interventions. Applying your social work expertise and communication skills, you’ll establish and maintain relationships with service users and agencies to meet their needs.

You’ll be committed to ensuring positive and sustainable outcomes. Using best practices, you’ll strive to provide the best results for adults in need. Your passion will be key in supporting vulnerable adults to thrive and flourish, with a priority on protection and efficiency.

Ideal candidate will be:

  • Qualified Social Worker registered with Social Work England
  • Experienced in working with vulnerable adults, including those with complex needs
  • Knowledgeable of adult social care legislation and safeguarding procedures
  • Skilled in assessment, planning, and review of care needs
  • Excellent communication, negotiation, and relationship-building skills
  • Committed to continuous improvement and high-quality service delivery
  • Experienced in multi-disciplinary team working

In this role you will:

  • Assess risks, needs, and protection requirements
  • Manage an agreed caseload
  • Develop individualized care plans promoting dignity and empowerment
