Qualified Social Worker, Adult Hospital Pathway Team

Pay rate: £30 per hour

Contract role

Vitalis is recruiting an experienced Social Worker for an Adult Hospital Pathway Team in Sutton. The role is based at St Helier Hospital, supporting patient discharges, assessments, and reviews in collaboration with the Continuing Healthcare team. Responsibilities include completing MCA assessments, conducting best interest meetings, managing complex and safeguarding cases, and carrying out Discharge to Assess (D2A) processes. The position requires presence at the hospital Monday to Friday, with no hybrid working.

Additionally, one social worker will be based at Civic Offices to support post-discharge Care Act assessments, safeguarding enquiries, and Court of Protection applications. This role offers hybrid working: 3 days in the office and 2 days remote.
