Job Title: Social Worker – Hospital Team

Location: Hackney

Rate: £32 per hour

An exciting opportunity has become available for an Adult Social Worker to join a Hospital Team based in Hackney.

Responsibilities:

  • Respond swiftly to service user needs using a multi-disciplinary assessment and care planning framework.
  • Respond to individuals in their own homes or normal residence; provide short-term bed-based interventions when appropriate.
  • Collaborate with individuals, carers, and other agencies to establish services that meet assessed needs and promote independence, choice, and control.
  • Operate within performance management and budgetary frameworks.
  • Manage caseloads, provide guidance, and support to colleagues and senior staff.
  • Adhere to standards set by the Health and Care Professional Council, maintaining registration and professional development.

Qualifications and Experience:

  • Social Work England registration.
  • Eligibility to work in the UK.
  • Degree-level qualification in Social Work.
  • Post-qualified experience.
