Job summary

Hackney Council is seeking a Brokerage Assistant for a 5-6 month contract to support the Council’s Brokerage function. The role involves managing databases of placements, support packages and costs, and working with care providers, social workers and health professionals to secure cost-effective care services.

You will record placements, keep vacancy data up to date and collaborate with Adult Social Care teams to ensure accurate information flow and compliance with policies.

Qualifications

• Experience in health or social care settings.
• Experience in finding placements for residential or supported living services.
• Experience in managing relationships with providers, agencies and stakeholders.
• Proficiency in IT systems for recording brokerage activities and services.
• Ability to work independently, manage workload and prioritise tasks.

Responsibilities

• Assist Brokerage Officers in sourcing, arranging and modifying placements in residential care, nursing homes, and supported living environments.
• Accurately record information about care packages, placements and costs on Mosaic, CM2000 and related systems.
• Keep the care home vacancy database current and accurate.
• Coordinate with social workers, care coordinators, contract officers, providers and finance teams to ensure accurate information flow.
