Role Description

This position provides professional advisory support to all referrers to the Community Equipment Service, ensuring residents' equipment needs are met and addressing issues identified by staff. The post involves working in partnership with health and social care colleagues to deliver effective services, supporting timely hospital discharges, and promoting early intervention and prevention of admissions.
