Zusammenfassung

The Forward Trust is seeking a full-time Supporting People on Probation Navigator in Colchester, Essex. You will manage a caseload, deliver tailored interventions, and support adults and young men transitioning from prison to the community.

You will work with probation teams and partner agencies, travelling as needed. This 35-hour role offers a salary of £26,000 per annum and enhanced DBS checks are required.

Qualifikationen

• Strong communication and relationship-building skills.
• A person-centred approach with understanding of challenges faced by people in the criminal justice system.
• Good organisational skills with ability to manage a caseload and meet deadlines.
• Confidence working collaboratively with a range of professionals and partner agencies.
• Flexibility to travel to other local projects when required, with occasional evening or weekend work.

Aufgaben

• Manage a caseload, completing assessments and developing personalised support and care plans.
• Deliver one-to-one interventions and group sessions focused on emotional wellbeing, relationships, lifestyle and social inclusion.
• Support individuals transitioning from prison into the community, working with partner agencies to promote continuity of care.
• Monitor progress, record outcomes and maintain accurate case management records.
• Identify and report safeguarding concerns, ensuring safety and wellbeing of those supported.
• Work collaboratively with colleagues and external agencies to achieve positive outcomes.
