Job summary

BWM PCN Limited is seeking an organised, compassionate RGN / Care Co-ordinator to support the Proactive Care Contract with WYICB in Leeds. You will coordinate resident reviews, engage with GP practices and community teams to deliver person-centred, proactive care, and support frailty services in care homes.

The role requires RGN registration, community experience, and strong SystemOne skills, with driving duties and a commitment to reducing hospital admissions.

Qualifications

• GCSE grade A–C in English and Maths.
• NMC registration (UK).
• Experience of working in the community.
• Excellent SystemOne knowledge.
• Access to a car with own insurance (mileage paid).
• Full driving licence.
• Care home experience.
• Computer literate and able to learn new systems.

Responsibilities

• Coordinate resident reviews and support MDT working.
• Identify patients eligible for Proactive Care using agreed criteria.
• Proactively contact patients to explain the programme and encourage engagement.
• Develop care plans and coordinate multidisciplinary care.
• Coordinate appointments and assessments with clinicians.
• Support care home staff with clinical advice and liaison with GP practices.
