Job summary

Betsi Cadwaladr University Health Board (BCUHB) is seeking a Substance Misuse Practitioner to plan, deliver and evaluate interventions for complex cases within HMP Berwyn. You will discuss treatment plans with service users and the wider SMS and Health team, and understand your role in delivering interventions within the plan.

You will identify precautions, monitor patients, alert your line manager to changes, support health promotion and harm reduction messages, and provide feedback on

Qualifications

• Qualified to degree level in a health-related field or equivalent experience.
• Working knowledge of Substance Misuse Practitioner role.
• Awareness of wider healthcare issues and delivery of substance misuse care.

Responsibilities

• Plan, deliver and evaluate interventions and/or treatments for complex cases.
• Discuss treatment plans with service users and the SMS/Health team.
• Identify precautions or contra-indications and take appropriate action.
• Monitor patients and alert line manager to changes in health or risk.
