Betsi Cadwaladr University Health Board (NHS) is seeking a Substance Misuse Liaison Practitioner (Band 4) to join the HMP Berwyn team in Wrexham. This permanent, full-time post is 37.5 hours per week, including weekends, within the prison health service.

You will plan, deliver and evaluate complex interventions, discuss treatment plans with service users and the SMS/Health team, identify precautions, monitor patients and support harm reduction and health promotion activities.

Competenze

• Qualified to degree level in a relevant subject or equivalent experience.
• Working knowledge of Substance Misuse Practitioner role.
• Awareness of wider healthcare issues and delivery of substance misuse care.

Mansioni

• Plan, deliver and evaluate interventions and/or treatments for complex issues.
• Discuss the treatment plan with the service user and the wider SMS and Health team.
• Identify any specific precautions or contra-indications to the proposed interventions/treatments.
• Support and monitor patients throughout, promptly alerting the line manager when there are changes in health or risk.
• Support the Psychosocial and clinical caseworkers to manage health promotion initiatives and harm reduction messages.
• Provide information on how individuals’ needs are changing and feedback on treatment plans when there are issues.

Formazione

Degree level in a relevant subject

Diploma level or equivalent experience in a health-related field
