Betsi Cadwaladr University Health Board (BCUHB) is seeking a Substance Misuse Liaison Practitioner for HMP Berwyn. The role involves planning, delivering and evaluating interventions for prisoners with complex needs, and collaborating with SMS and Health teams to implement the treatment plan.

The post values experience in substance misuse care within challenging settings, with on-site duties at the prison. Welsh language skills are desirable, and applications in Welsh are welcome.

Qualificações

• Diploma level or equivalent experience in a relevant health related field or equivalent experience.
• Working knowledge of Substance Misuse Practitioner role.
• Awareness of wider healthcare issues and delivery of substance misuse care.

Responsabilidades

• Plan, deliver and evaluate interventions and/or treatments when there are complex issues.
• Discuss treatment plan and related needs with the service user and the wider SMS and Health team.
• Identify specific precautions or contra-indications to proposed interventions/treatments and take appropriate action.
• Support and monitor patients, alerting line manager to changes in health or risk.
• Support psychosocial and clinical caseworkers to manage health promotion and harm reduction messages.
• Provide information to Substance Misuse team on changing needs and feedback on treatment plans.
