Job summary

Change Grow Live is seeking a Female Prison In-Reach Recovery Worker in Southall. The role focuses on engaging service users with drug and alcohol needs, delivering targeted interventions, and ensuring robust release planning to reduce substance misuse and re-offending.

You will work with prisons, probation and community providers to ensure seamless transitions on release, with duties including screenings, risk assessments, and recovery planning.

Qualifications

• Empathy and understanding of substance misuse issues within Criminal Justice settings.
• Clear verbal and written communication; proficient IT skills.

Responsibilities

• Act as liaison between prison in-reach teams and community services to support continuity of care.
• Carry out prison-based screenings and initial risk assessments, developing recovery plans with service users before release.
• Provide harm reduction education, overdose prevention and relapse planning for custody and post-release.
• Deliver and promote Naloxone provision pre- and post-release, training users where appropriate.
• Conduct home and hostel visits, and community outreach to maintain engagement with high-risk individuals.
• Collaborate with police, probation, mental health, housing and social care partners.
