Phoenix Futures is recruiting a Through the Gate worker to support individuals with substance use issues at HMP Pentonville, delivering harm reduction, relapse prevention and recovery planning. You will advocate for service users and coordinate with internal and external agencies to support successful outcomes.

The role is based at the prison, with a 75-hour fortnight schedule and a rota including evenings/weekends; the team ensures safety protocols and staff support for these assignments.

Qualificações

• Experience working with vulnerable adults presenting with substance, alcohol and associated issues.
• Able to work on own initiative and as part of a team.
• Good communication and organisational skills.
• Flexible to work late shifts and weekends on a rota.

Responsabilidades

• Provide harm reduction and relapse prevention support to service users.
• Support in recovery planning and signpost to community agencies.
• Prepare service users for release and accompany to community appointments when needed.
• Advocate for service users and work with internal/external partners.
