This exciting new post has been developed to improve diabetes care and outcomes for young people (age 16-25) with diabetes across North East London. The post holder will support young adults who may be struggling with a range of difficulties related to living with diabetes, as they make the transition to adult services. The work will mainly be with young adults living with Type 1 diabetes but there will also be scope to develop a pathway for young adults living with Type 2 diabetes.

As this is a new post, the post holder will have scope to develop the service, with the support of more senior colleagues and in collaboration with service users, and to take a leading role in the future of diabetes services for young adults across North East London.

The post will have a base at Mile End Hospital, but will involve working across all of the Barts Health sites, and ensuring that standardised, high quality care is delivered across the Trust.

This varied post will involve psychological assessment and formulation, individual work, group work and consultation. There is also scope to supervise other members of the Young Adults Diabetes Service.

Main duties of the job

To work as part of the core young adults diabetes team across Barts Health to improve diabetes care and outcomes to all young people (age 16-25) with diabetes in North East London.

To work with the wider diabetes care pathway across primary care and mental health services in Tower Hamlets, Waltham Forest, City & Hackney and Newham, in order to provide joined up and effective psychological care to people with diabetes locally.

To provide a comprehensive psychology assessment and intervention service for young people with all type 1 or type 2 diabetes and support their transition from paediatric care to adult diabetes care and there on up to the age of 25.

The role will include working across all of the Barts Health sites and ensuring that standardised, high quality care is delivered across the trust.

Across all sites, to take the developmental lead, and be accountable for the direct provision of an autonomous and systematic specialist clinical service comprising specialised assessments, a range of psychological interventions appropriate to individuals who have Type 1 diabetes and support to the MDT, individuals, couples, careers and families, as appropriate.

To develop new ways of working, which could include group based psychological interventions. There will therefore be opportunities to develop leadership skills, as well as teaching and training skills.
