Job summary

Surrey Heath Community Providers Limited is seeking a Pharmacy Technician to join the Clinical Pharmacy Team across General Practices and Care Homes in Surrey Heath. You will support clinical pharmacists and the MDT by conducting resident-facing activities, medicines reconciliation, and structured reviews under supervision, while helping implement network contracts and PCN responsibilities.

You will build EMIS reports, liaise with community pharmacists, and develop medicines management

Qualifications

• GCSEs including English and Maths (C or above)
• Registration with GPhC
• Completed approved primary care educational pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH)
• Experience in medicines management or community pharmacy
• Experience in General Practice repeat prescribing policies and systems
• Ability to analyse and present prescribing activity data
• Experience of multidisciplinary working
• Ability to apply evidence-based medicine to prescribing

Responsibilities

• Carry out medicines optimisation tasks to improve prescribing quality and patient adherence
• Build and run EMIS reports to identify residents for review
• Undertake medicines reconciliation and communicate changes
• Support GP practices and PCN in medicines optimisation
• Assist with structured medication reviews (SMRs) across care homes and general practices
• Synchronise medicines during patient transfers between care settings
• Provide patient-facing support and ensure correct inhaler technique
• Contribute specialist knowledge to address public health and social care needs
