Job summary

NHS Scotland's Orkney health system seeks a Primary Care Clinical Pharmacist to join the Polypharmacy and Medicines Reablement Service across Orkney, including remote and island communities.

The post involves structured polypharmacy reviews, medicines reconciliation and medicines optimisation, working with GP practices, care homes and patients to reduce medicines-related harm and maintain independence for longer. It offers flexible hybrid working.

Qualifications

• Registered pharmacist with excellent clinical, communication and interpersonal skills.
• Experience in primary care, community pharmacy or hospital pharmacy welcomed.
• Independent Prescribing qualification is essential.

Responsibilities

• Undertake structured polypharmacy reviews and medicines reconciliation.
• Lead medicines optimisation activities to reduce harm and support independence.
• Collaborate with GP practices, care homes and hospital teams; use digital tools for remote care.
• Contribute to service development and innovative models of care across Orkney.
