Job summary

NHS Orkney is seeking a motivated Pharmacist to join the Primary Care Pharmacy Team for a new Polypharmacy and Medicines Reablement Service across Orkney. The post involves remote clinical practice and planned visits, with a hybrid working approach and a fixed-term/secondment until November 2028.

The successful candidate will contribute to structured polypharmacy reviews, medicines reconciliation and optimisation across GP practices, care homes and patients at home, ensuring person-centred

Qualifications

• Registered pharmacist with excellent clinical, communication and interpersonal skills.
• Independent prescribing qualification is essential.
• Experience in primary care, community pharmacy or hospital pharmacy is welcomed.

Responsibilities

• Undertake structured polypharmacy reviews and medicines reconciliation.
• Deliver medicines optimisation activities to reduce medicines-related harm.
• Collaborate with GP practices, care homes, hospital teams and community services.
• Provide remote and in-person pharmaceutical care using digital tools (NHS Near Me, Microsoft Teams).
