Job overview: We are seeking mental health professionals from a registered core profession (RMN, OT, Social worker) to join the Primary Care Mental Health Team within the Leeds Mental Wellbeing Service, in partnership with statutory and third sector services. This post offers hybrid working, involving GP surgeries, community mental health team bases, and home working within a locality hub. The team adopts a transdiagnostic approach, focusing on helpful conversations and biopsychosocial assessments to identify and address patient difficulties.
We work with patients to explore their experiences, set goals, and determine supportive interventions, including referrals, psychoeducation, and practical coping strategies. We value diverse skills and professional interests, and the successful candidate will work autonomously and within a team, with training and supervision provided.
Main duties of the job: The Locality Primary Care Mental Health Practitioners will support service users with complex mental health issues by providing assessments, risk evaluations, and brief interventions to prevent deterioration. They will also assist primary care colleagues with triage, assessment, and decision-making to ensure appropriate care pathways, supporting Leeds' mental health care framework under the Community Mental Health Transformation Project.
