Job overview

We are seeking mental health professionals from a registered core profession (RMN, OT, Social worker) to join us for an exciting opportunity to be part of the ongoing development of the Primary Care Mental Health Team within the new Leeds Mental Wellbeing Service, in partnership with both statutory and third sector services. This post offers hybrid working and will involve a mixture of working within GP surgeries, community mental health team base, and home working within a locality hub.

The team takes a transdiagnostic approach to mental health care, with the aim of using helpful conversations and biopsychosocial assessments to identify and formulate the patient’s difficulties. We work alongside the patient to explore their current experiences, identify goals and priorities, and consider what support may be helpful to achieve these. This can include identifying existing services, supporting referrals, psychoeducation, and practical coping strategies for symptom management.

We value unique skills, specialisms, and professional interests. The successful candidate will work autonomously and as part of a team, with appropriate training and support, including clinical supervision.

Main duties of the job

The Locality Primary Care Mental Health Practitioners will be linked to a locality hub, providing a specialist mental health clinical role to support service users with complex mental health issues. They will provide mental health assessments, risk assessments, and brief interventions to prevent deterioration requiring secondary care referral. They will also offer triage, assessment, and decision support to primary care colleagues to ensure appropriate pathways and interventions.

This role supports Leeds’ vision for developing a new mental health framework, integrating specialist mental health knowledge into primary and community settings under the Community Mental Health Transformation Project.

Detailed job description and main responsibilities

  • Ensure effective communication systems are in place and used appropriately.
  • Participate in meetings and engage positively with service users and carers.
  • Develop networks with other agencies providing services to those with complex mental health needs.
  • Report to the Band 6 Locality Primary Care Mental Health Specialist and be accountable to the Locality Manager.
  • Adapt communication to barriers presented by service users, utilizing high-level communication skills.
  • Maintain current professional and legal standards per professional codes of conduct.
  • Work in partnership with service users, carers, and other agencies.
  • Undertake specialist mental health and risk assessments, sharing skills with team members.
  • Contribute to service development initiatives and reduce mental health inequalities by applying knowledge of social determinants.
  • Provide advice, support, and information to service users and carers.
  • Develop links with partner agencies.
  • Supervise junior staff and students when appropriate.
  • Offer consultation, advice, and decision support to primary care colleagues.
  • Use expert judgment and analytical skills in clinical practice and decision-making, in line with professional codes.
  • Prioritize and manage workload effectively under pressure.
