Job summary

NHS Greater Glasgow and Clyde is seeking a Pharmacist (Primary Care) to deliver patient-centred pharmaceutical care across primary care settings, working as part of a multidisciplinary team to support safe prescribing and medicines optimisation.

This permanent, full-time role is based at Port Glasgow Health Centre, with hours Monday to Friday, 08:30 to 16:30, and opportunities for professional development within Scotland's largest NHS Board.

Qualifications

• Masters Degree in Pharmacy or equivalent qualification.
• Registration with the General Pharmaceutical Council.
• Commitment to NES Foundation Pharmacist Training Programme.
• Knowledge of clinical pharmacy practice and medicines management principles.

Responsibilities

• Deliver clinical pharmacy services to patients within GP practices, care homes and primary care settings.
• Undertake medication reviews to ensure medicines are safe, clinically appropriate and cost effective.
• Provide specialist medicines advice and support to GPs, nurses, community pharmacists and other healthcare professionals.
• Support prescribing governance, formulary compliance and medicines optimisation initiatives.
• Respond to medicines-related enquiries and contribute to safe prescribing and medicines reconciliation processes.
• Educate and support patients, carers and healthcare professionals on the safe and effective use of medicines.
• Participate in service improvement, audit, quality improvement activities and multidisciplinary team working.
