Job summary

Leaders in Care is seeking a Social Worker for the Preparing for Adulthood team in the UK. You will support young people with additional needs as they transition to adult services, helping families plan for education, employment, health, housing and community inclusion.

The role is hybrid, with ongoing professional development and a loyalty bonus, offering a rewarding opportunity to deliver person-centred transitions that promote independence and positive long-term outcomes.

Qualifications

  • A recognised Social Work qualification
  • Current Social Work England registration
  • Experience supporting young people with disabilities or complex needs
  • Knowledge of transition planning and adulthood pathways
  • Strong assessment, care planning and partnership-working skills
  • Person-centred, strengths-based approach to practice

Responsibilities

  • Work with young people approaching adulthood, families and multi-disciplinary partners to complete assessments
  • Develop person-centred plans and coordinate support for a successful transition

We’re looking for an experienced Social Worker to join a Preparing for Adulthood team. You’ll support young people with additional needs and disabilities through the transition to adult services, helping them and their families plan for the future with confidence.

The role

You will work with young people approaching adulthood, their families and multi-disciplinary partners to complete assessments, develop person-centred plans and coordinate the support needed for a successful transition. This will include supporting planning around education, employment, health, housing, independence and community inclusion.

What you’ll need
  • A recognised Social Work qualification
  • Current Social Work England registration
  • Experience supporting young people with disabilities, additional needs or complex needs
  • Knowledge of transition planning and preparing for adulthood pathways
