We’re looking for an experienced Social Worker to join a Preparing for Adulthood team. You’ll support young people with additional needs and disabilities through the transition to adult services, helping them and their families plan for the future with confidence.
You will work with young people approaching adulthood, their families and multi-disciplinary partners to complete assessments, develop person-centred plans and coordinate the support needed for a successful transition. This will include supporting planning around education, employment, health, housing, independence and community inclusion.
