Job description

The post holder will be responsible for assessing, screening, treating and advising and educating patients,working as part of the primary health care team.

They will deliver a wide range range of defined patient care, including basic treatment room care, chronic disease management and contributing to health care promotions.

Main duties of the job

Key responsibilities of the role include

Patient consultations

Performing clinical procedures such as phlebotomy, administering injections, wound care.

Chronic Disease Management. Working with patients to mange conditions such as asthma and diabetes through regular reviews and education.

Health Promotion: Promoting healthy life styles, providing family planning advice, immunisations etc.

Collaborating with GPs to provide high quality care for patients.

Health education and advice to patients on a range of topics, such as healthy living and disease prevention.

Chronic disease management, Asthma, Diabetes, COPD etc.

Cervical cytology

Baby and Childhood immunisations

Wound care

ECG

Immunisations

Travel health

Ordering of vaccines

Supervising the Phlebotomist

Organising the Seasonal Flu campaign

Supporting the practice in contractually meeting clinical targets from NHS Wales

Ensuring compliance with relevant legislation, regulations and quality standards

Maintaining accurate patient records and ensuring smooth flow of information within the practice.

Adhering to the NMC Code of Professional Conduct and practice protocols.

Qualifications
  • Registered Nurse with valid registration with the Nursing and Midwifery Council
