A healthcare provider in Gaerwen is seeking a Registered Nurse to join their team. The role involves assessing and treating patients, managing chronic diseases, and conducting health education. Candidates should have valid nursing registration and experience in primary care. The position offers part-time hours and requires strong communication and interpersonal skills.
The post holder will be responsible for assessing, screening, treating and advising and educating patients,working as part of the primary health care team.
They will deliver a wide range range of defined patient care, including basic treatment room care, chronic disease management and contributing to health care promotions.
Key responsibilities of the role include
Patient consultations
Performing clinical procedures such as phlebotomy, administering injections, wound care.
Chronic Disease Management. Working with patients to mange conditions such as asthma and diabetes through regular reviews and education.
Health Promotion: Promoting healthy life styles, providing family planning advice, immunisations etc.
Collaborating with GPs to provide high quality care for patients.

Ensuring compliance with relevant legislation, regulations and quality standards
Maintaining accurate patient records and ensuring smooth flow of information within the practice.
Adhering to the NMC Code of Professional Conduct and practice protocols.
