Modality East Surrey Medical Practice is seeking a skilled Practice Nurse to deliver evidence-based care across four sites in Horley, working within NMC standards and approved protocols. You will support safe, coordinated care and contribute to audit and service improvement, with opportunities for teaching and mentorship based on your experience.

The practice serves around 45,000–46,000 patients and offers a broad scope of practice nursing within a forward-thinking training environment.

Responsabilités

• Nurse-led reviews and support for long-term conditions, including diabetes, asthma and COPD, according to training and competence.
• Cervical screening, immunisations and vaccinations with appropriate training and authorisation.
• Wound assessment, dressings, suture removal and other routine nursing procedures within competence.
• Preventative care, screening and health checks, with personalised lifestyle and self-management advice.
• Recognition and escalation of deterioration and safeguarding concerns.
• Accurate clinical documentation, infection prevention and safe handling of medicines and equipment.
• Collaboration with specialist nurses, GP partners and the wider multidisciplinary team.
• Participation in audit, quality improvement, continuing professional development and sharing learning.
