An NHS practice in Wareham is seeking a Practice Nurse to deliver essential healthcare services to patients with long-term conditions. The ideal candidate will be responsible for chronic disease management and infection control, implementing evidence-based practices, and working collaboratively within a multidisciplinary team. This role offers the opportunity to enhance patient care in a supportive environment.
To be responsible for the implementation of processes for theeffective management of patients with long-term conditions using evidence-basedpractice including care for elderly and housebound patients.
Working as a senior member of the practice multidisciplinary team,the post holder will ensure nursing services are delivered effectively to theentitled patient population.
The Practice Nurse will be responsible for a number of clinicalareas such as infectionprevention and control, health promotion, chronicdisease management, health promotion, well women and well man clinics, as wellas actively supporting the practice management team in the reviewing anddelivery of clinical policy and procedure.
The coreresponsibilities of the Practice Nurse are as follows. On occasion, there maybe a requirement to carry out tasks not listed below, that are within thereasonable scope of the role.
To supportthe Infection Prevention Control (IPC) lead and to work to the IPC Policy
To assessthe needs of patients ensuring the provision of healthcare is appropriate,incorporating evidence based practice
Implementand evaluate individual treatment plans for chronic disease patients thatpromote health and wellbeing
Toidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients health
Provideroutine nursing care to patients in accordance with clinical based evidence,NICE and NSF
Todiagnose and manage acute and chronic conditions as required
To reviewpatient medications to enhance compliance
Co-ordinatethe provision of travel medicine services
Maintainaccurate clinical records in conjunction with existing legislation
Ensureread codes are used effectively by all member of the nursing team
Ensureappropriate use of the chronic disease registers
Prioritisehealth issues and intervene appropriately
To supportthe team in dealing with clinical emergencies
Recognise,assess and refer patients presenting with mental health needs
Toimplement vaccination programmes for adults and children
Sandford Surgery is a small , friendly NHS practice located in the Parish of Sandford, about a mile from Wareham and7 miles from Poole.
In addition to thecore responsibilities, the Practice Nurse may be requested to;
1.Developand review audit protocols and process for the practice, effectively utilising theaudit cycle
2.Develop,review and implement nursing protocols in conjunction with Lead Practice Nurse/NationalNursing Lead
3.Supportall members of the nursing team, providing guidance when necessary
4.Participatein local initiatives to enhance service delivery and patient care
5.Supportand participate in shared learning within the practice in order to improvepatient care
6.Continuallyreview clinical practices, responding to national policies and initiativeswhere appropriate
7.Participatein the review of significant and near-miss events applying a structuredapproach
8.Coordinatethe nursing team rota, ensuring sufficient staff are available to meet patientsneeds
9.Lead theplanning and implementation of changes withing the nursing team, enhancing theprovision of services to patients
10.Supportthe practice team with the reviewing and implementation of practice policiesand protocols, ensuring conformance to existing legislation
