Job summary

The Trafalgar Medical Group Practice is seeking an experienced Practice Nurse to deliver evidence-based general practice nursing to the whole patient population. The role focuses on long-term condition management and preventative care, with autonomous practice and leadership within the practice team.

You will work within a 27,000-patient list across 2 sites, collaborating with the GP partners to meet patient needs, support policy delivery, and contribute to nurse leadership as required.

Qualifications

• Registered General Nurse.
• Commitment to professional development.
• Experience with the UKCC Code of Professional Conduct and The Scope of Professional Practice.
• Basic computer/typing skills.
• Experience dealing with the public / patients.
• Competent in basic nursing duties required for the post.
• Primary care experience desirable, but other suitable backgrounds considered.
• Experience in Primary Care.

Responsibilities

• Assess, plan, implement and evaluate treatment plans for patients with a known long-term condition.
• Proactively identify and manage treatment plans for patients at risk of developing a long-term condition.
• Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.
• Work with patients in order to support adherence to prescribed treatments.
• Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
• Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
• Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care.
• Support and manage health needs of women presenting for family planning and cervical cytology consultations.
• Recognise and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
• Implement and participate in vaccination and immunisation programmes for both adults and children.
• Advise, support and administer vaccinations for patients travelling abroad.
• Meet the needs of patients presenting for opportunistic wound care.

The Trafalgar Medical Group Practice is a 27,000 patient list, 6 GP Partner Practice situated in the heart of Southsea. The Practice operates across 2 sites. The Practice is also the lead Practice for the Portsmouth South Coast Primary Care Network.
This is an exciting opportunity and time to join the nursing team in a forward thinking, developing, busy, and diverse GP Practice. This is an established training Practice and we are looking for an experienced, enthusiastic and motivated Practice Nurse who can integrate well and who has skills to offer which can benefit our patients.
Previous experience within Primary Care would be desirable, but we will also consider applications from nurses whose background will also provide suitable experience and qualifications.
Hours are negotiable, but there will be a requirement to cover staff absences when required.
Salary - Competitive, subject to qualifications and experience.
