Job summary

The Mount View Practice in Fleetwood, Lancashire is seeking an enthusiastic Practice Nurse to join our patient-focused primary care team. 20 hours per week (Mondays & Fridays) with training and development support available to upskill in chronic disease management, cytology, vaccinations, and more.

We welcome applicants from experienced nurses as well as those with transferable skills from other backgrounds who are keen to develop in primary care.

Qualifications

• Active NMC registration and practising Certificate
• Experience in primary care or community nursing preferred
• Immunisations qualifications or willingness to train to provide vaccines

Due to retirement, we are seeking an enthusiastic and motivated Practice Nurse to join our friendly, patient-focused primary care team. This is an excellent opportunity to work within a supportive and forward-thinking GP practice committed to delivering high-quality care to our local population.
We are ideally looking for an experienced Practice Nurse; however, we would also warmly welcome applications from nurses with a different clinical background who can demonstrate transferable skills and a willingness to develop within primary care.
The successful candidate will play a vital role in delivering nursing services, health promotion, and chronic disease management within a busy but rewarding environment.
Hours: 20 hours per week (Mondays & Fridays essential)

Job responsibilities
Job Purpose
To provide high-quality, patient-centred nursing care within the practice, supporting the delivery of primary care services, chronic disease management, health promotion, and treatment room services.
The post holder will work collaboratively with the multidisciplinary team to ensure safe, effective, and efficient care for the practice population.
Key Responsibilities
Clinical Duties

• Deliver high-quality, evidence-based nursing care to patients within the practice setting
• Lead and contribute to chronic disease management clinics (e.g. diabetes, COPD, asthma), in line with local and national guidelines
• Undertake cervical cytology (smear tests) in accordance with national screening programmes
• Administer immunisations and vaccinations in line with current protocols and Patient Group Directions (PGDs)
• Provide wound care, dressings, and a full range of treatment room services
• Assess, plan, implement and evaluate patient care
• Promote health and wellbeing through patient education and lifestyle advice
• Support national and local health promotion initiatives
• Encourage patients to engage in self-care and preventative health programmes

Infection Prevention & Control

• Act in accordance with infection prevention and control policies
• Maintain a safe clinical environment for patients and staff
• Participate in audits and compliance activities where required

Administrative & Record Keeping

• Maintain accurate, contemporaneous, and legally compliant clinical records
• Use clinical systems (e.g. EMIS) effectively to support patient care
• Contribute to data collection and quality improvement initiatives (e.g. QOF)

Team Working & Communication

• Work collaboratively with GPs, nurses, healthcare assistants and the wider multidisciplinary team
• Communicate effectively with patients, carers, and colleagues
• Participate in practice meetings and contribute to service development

Professional Responsibilities

• Maintain active NMC registration and adhere to the NMC Code
• Engage in continuing professional development (CPD)
• Participate in appraisal and revalidation processes
• Work within own scope of competence and seek support when required

Training & Development

• Participate in a structured induction programme
• Engage in ongoing training including chronic disease management, cytology, and immunisations (if required)
• Access mentorship and clinical supervision
• Take part in internal and external CPD opportunities
